- Good Standing With the Kentucky Board of
Dentistry
- No Board of Dentistry Disciplinary Action In
Past 3 years
- Practicing for at least 3 years
- Currently holding active Kentucky Dental
License
- Currently holding active Malpractice
Insurance Policy
- Completion of Gratis Appointment
Application with the University of Louisville
School of Dentistry
